La punción percutánea de la vena sube/avía para hemodiálisis transitoria provoca una frecuencia elevada de obstrucciones venosas proximales, que interfieren con la posterior realización de un angíoacceso en ese miembro superior.
Se presentan ocho casos de oclusión sube/avía; cinco de ellos demostrados en pacientes asintomáticos mediante la realización preoperatoria de flebografía y Duplex venoso.
Dada la importancia de esta complicación se propone el cateterismo preferencial de la vena yugular para la hemodiálisis transitoria, el uso de catéteres menos trombogénicos y el estudio preoperatorio mediante flebografía o Duplex venoso de los pacientes que hayan tenido catéteres colocados por punción subclavia.

Se analizan los resultados de 21 procedimientos de recuperación de fístulas arteríovenosas (FAV) protésicas trombosadas en 10 pacientes, obteniendo un alto porcentaje global de rescate de las FAV (76%), a pesar del tiempo de evolución prolongado de la trombosis en algunos casos.
La efectividad fue mayor cuando la trombectomía se acompañó de la corrección de un defecto anatómico topografiado en general a nivel de la anastomosis venosa.
Se destaca la importancia de la identificación de los defectos anatómicos en la FAV, fundamentalmente con flebografía intraoperatoria, y su tratamiento concomitante con la trombectomía.

Ante la necesidad de obtener modelos experimentales prácticos para la obtención de lesiones de hiperplasia intima, los autores comunican la experiencia del Departamento Básico de Cirugía en el desarrollo de un modelo de lesión arterial en perros, por medio de la
dilatación arterial con sonda Foley.
Se intervinieron 20 animales, dividiéndolos en series según la evolución de 7, 1 O y 15 días.
Las arterias fueron estudiadas del punto de vista macro y microscópico, obteniéndose lesiones de hiperplasia intima en la serie con evolución de 15 días.

A través de un estudio clínico, los autores demuestran la posibilidad de dejar abiertas las heridas quirúrgicas del recto subperitoneal, sin infección ni trastornos de cicatrización. Este hecho lo corroboran a través de un estudio experimental en el perro. No se aclara sin embargo la explicación del porqué no se produce infección alguna, planteándose como hipótesis no demostrada la protección creada por la dilatación anal y en el perro el hecho de dejar la herida abierta alcanza para su protección. Se plantea la posibilidad de futuro de cambio de conducta en las heridas traumáticas del recto subperitoneal.

Se realiza el análisis de 85 pacientes con síntomas de insuficiencia arterial periférica, comparando el estudio hemodinámico Doppler con la arteriografía.
Con este fin cada miembro es dividido en 3 segmentos: aortoilíaco, fémoro-poplíteo y poplíteo distal. En los 320 segmentos estudiados con Doppler se comprueba una sensibilidad del 92%, una especificidad del 97% y una seguridad diagnóstica elevada, revelada por un índice de Youden de 0,89, por lo que el Doppler se convierte en un estudio de primera línea para el paciente vascular periférico.

Se realizan consideraciones etiopatogénicas sobre el linfedema. Se destaca, la función de válvula en el sentido linfodinámico de la corriente linfática de los linfonódulos y de la cisterna de Pecquet.
Se distingue el reflujo del quilo del reflujo linfático pélvico. Bajo este concepto se plantea el tratamiento medico quirúrgico de esta afección. Se destaca la importancia de la linfografía en el diagnóstico de esa enfermedad. Se presenta un caso de nuestra casuística y los resultados obtenidos hasta los tres meses de operado.
