Ante la necesidad de obtener modelos experimentales prácticos para la obtención de lesiones de hiperplasia intima, los autores comunican la experiencia del Departamento Básico de Cirugía en el desarrollo de un modelo de lesión arterial en perros, por medio de la
dilatación arterial con sonda Foley.
Se intervinieron 20 animales, dividiéndolos en series según la evolución de 7, 1 O y 15 días.
Las arterias fueron estudiadas del punto de vista macro y microscópico, obteniéndose lesiones de hiperplasia intima en la serie con evolución de 15 días.

El procedimiento nos dio buenos resultados, obteniendo en doce pacientes fistulas arteriovenosas funcionantes y utilizables en hemodiálisis crónica.

Se presenta un caso de obstrucción embólica del tronco tibioperoneo luego de la introducción de catéter de Seldinger en arteria femoral sana, para estudio circulatorio cerebral. El enfermo, luego de un severo cuadro agudo de isquemia, logró mantener el trofismo de su miembro, aunque quedó con severa claudicación intermitente. Un estudio arteriográfico mostró obstrucción completa del tronco tibioperoneo y de la tibial anterior en su origen. El tratamiento consistió en la realización de un injerto libre con vena safena interna desde la arteria poplítea baja a la arteria tibial posterior en parte media de pierna, con excelente resultado clínico y arteriográfico.

Los autores reportan un caso de obstrucción ureteral bilateral por aneurisma aorto-ilíaco con insuficiencia renal, tratado quirúrgicamente con reemplazo vascular y liberación ureteral, con buena evolución inmediata clínica y radiológica. Estudian los casos presentados en la literatura hasta el presente. Destacan la importancia de la correcta valoración nefro-urológica preoperatoria en la cirugía reglada de la aorta abdominal y en especial del aneurisma.
Consideran como tratamiento de elección el reemplazo arterial con liberación ureteral.

Se estudian 4 casos de obstrucción o severa estenosis de un grueso tronco supraaórtico sobre 130 casos de cirugía carotídea o subclaviovertebral. En todos los casos se utilizó un procedimiento de revascularización por vía cervical o cérvico-axilar. En 3 casos se utilizó bypass extraanatómico de dacrón, uno de ellos compuesto ( subclavio-subclaviocarotídeo). Los enfermos no presentaron morbilidad ni mortalidad y curaron de su sintomatología. Se insiste en la utilidad de la vía cervical y la posibilidad de tratamiento de revascularización independientemente de la etiología.

La punción percutánea de la vena sube/avía para hemodiálisis transitoria provoca una frecuencia elevada de obstrucciones venosas proximales, que interfieren con la posterior realización de un angíoacceso en ese miembro superior.
Se presentan ocho casos de oclusión sube/avía; cinco de ellos demostrados en pacientes asintomáticos mediante la realización preoperatoria de flebografía y Duplex venoso.
Dada la importancia de esta complicación se propone el cateterismo preferencial de la vena yugular para la hemodiálisis transitoria, el uso de catéteres menos trombogénicos y el estudio preoperatorio mediante flebografía o Duplex venoso de los pacientes que hayan tenido catéteres colocados por punción subclavia.
