Diecinueve by-pass a los sectores distales de los troncos de pierna fueron realizados, todos ellos con vena safena autóloga invertida. El angiograma preoperatorio se considera fundamental para indicar la cirugía, que se contraindica en ausencia de arco plantar. La permeabilidad al alta hospitalaria fue del 84 %. El seguimiento fue de 3 meses a 8 años. Hubo 3 obstrucciones alejadas, seguramente por fibroplasia intimal.

El estudio presenta como objetivo valorar el impacto en la calidad de vida en una muestra de pacientes portadores de amputaciones de pie diabético asistidos en la Unidad de Pie Diabético del Hospital Pasteur, Montevideo en un periodo comprendido entre agosto y
septiembre de 2018. Se aplicó el cuestionario DQOL que determina la calidad de vida a 54 pacientes. También se indagó sobre las amputaciones y sobre los ámbitos socio-económico, cultural y familiar. Los resultados mostraron mayor participación del sexo masculino (74,1%). El 33% de los pacientes padecían diabetes Tipo 1 y 66% Tipo 2.
La edad media del diagnóstico fue a los 41,5 años. Las amputaciones menores incluyen la amputación digital (74%), en el caso de las amputaciones mayores se destacan las amputaciones de pierna (85,7%).
Se pudo determinar que la calidad de vida en pacientes con pie diabético fue regular. Se observaron diferencias en la calidad de vida en los pacientes con diabetes Tipo 1 respecto a los pacientes Tipo 2, concomitantemente, mayor afectación de la calidad de vida en el grupo con amputación mayor en relación a amputación menor. No se observó diferencia en la calidad de vida entre pacientes con una amputación respecto a los que presentan amputación bilateral. Se encontró un deterioro en la calidad de vida en los pacientes con mayor tiempo transcurrido entre el diagnóstico de diabetes y la primera amputación.

Se presenta una observación de cirrosis hepática con hipertensión portal y hemorragia digestiva grave por ruptura de várices esofagogástricas, en una enferma que cursaba el 8 mes de embarazo. Se practicó cesárea ( extracción de feto muerto) y anastomosis porto-cava terminolateral en el mismo acto operatorio. Se analiza la asociación de cirrosis-embarazo y la táctica quirúrgica seguida.

Resumen
Se realizó revascularización de las arterias del pie a 20 pacientes predominantemente diabéticos que presentaban isquemia del pie, con lesiones tróficas 90% y lo dolor de reposo 60%.
Todos tenían pulso femoral y el 40% poplíteo, ninguno tenía pulso tibia/ posterior ni pedio.
La severidad de las alteraciones hemodinámicas fueron valoradas por el Doppler y la fotopletismografía. Todos presentaban oclusión de los ejes de pierna. Se logró 80% de resultados favorables, con preservación del pie para la deambulación en lo inmediato. En el control evolutivo entre 1 y 36 meses se ha ocluido un solo puente, habiendo fallecido 5 pacientes por causas cardíacas.
