La enfermedad arterial periférica de los miembros inferiores es una de las principales causas de morbilidad cardiovascular. Presenta una elevada prevalencia tanto a nivel nacional como internacional. En etapas avanzadas puede condicionar la pérdida de la extremidad afectada. El objetivo del presente trabajo es evaluar el éxito clínico de las angioplastias de ejes distales de pierna en pacientes intervenidos en el Hospital de Clínicas Dr. Manuel Quintela, durante el período 2016-2022.

El pasado 27 de marzo, la Sociedad Uruguaya de Cirugía Vascular y Endovascular celebró su primera reunión de socios del año 2025, con una asistencia de más de 40 participantes.<br /><br />El evento fue un espacio de intercambio y actualización, donde se presentaron importantes novedades para los socios:<br /><br />
<ul>
<li>Lanzamiento de la nueva página web: Una plataforma digital moderna y accesible que facilitará la comunicación y el acceso a información relevante para los miembros.</li>
<li>Presentación del nuevo logo: Una imagen renovada que refleja la identidad y los valores de la sociedad.</li>
<li>Introducción a la biblioteca digital: Un recurso en línea con acceso a publicaciones y materiales de interés para la especialidad.</li>
<li>Nuevo canal de YouTube: Un espacio audiovisual para compartir contenido de interés, entrevistas con expertos y actualizaciones relevantes para la comunidad.</li>
<li>Nuevo servicio bibliotecológico de localización de artículos a demanda y búsquedas bibliográficas: Un servicio personalizado que facilitará el acceso a la información científica más actualizada, optimizando el tiempo de los miembros.</li>
</ul>
<br />La reunión comenzó con una bienvenida a cargo del actual presidente, el Dr. Omar Rudnitzky, quien destacó la importancia de este encuentro para fortalecer la comunidad de cirujanos vasculares y endovasculares en Uruguay.<br /><br />A continuación, la Lic. Cecilia Valenzuela realizó una presentación detallada de la biblioteca digital, explicando sus funcionalidades y beneficios.<br /><br />El evento concluyó con la exposición de la Dra. Gabriela Aguiar, de la Subcomisión de Radioprotección, quien abordó el tema de la exposición a radiaciones ionizantes en cirujanos vasculares. <br /><br />Este encuentro marcó un hito importante para la Sociedad Uruguaya de Cirugía Vascular y Endovascular, consolidando su compromiso con la actualización y el desarrollo profesional de sus asociados.

Se realiza el análisis prospectivo de 60 pacientes con síntomas de insuficiencia arterial periférica de miembros inferiores, comparando la clínica, la arteriografía y el estudio hemodinámico por el efecto Doppler.
En el mismo se destaca la importancia del estudio Doppler como un medio semiológico más, que incorporado al estudio funcional del paciente y analizándolo conjuntamente con la arteriografía, se convierte en un aliado invalorable para el diagnóstico de los enfermos vasculares, la decisión terapéutica, el pronóstico funcional y su seguimiento.

Diecinueve by-pass a los sectores distales de los troncos de pierna fueron realizados, todos ellos con vena safena autóloga invertida. El angiograma preoperatorio se considera fundamental para indicar la cirugía, que se contraindica en ausencia de arco plantar. La permeabilidad al alta hospitalaria fue del 84 %. El seguimiento fue de 3 meses a 8 años. Hubo 3 obstrucciones alejadas, seguramente por fibroplasia intimal.

Análisis de un planteo terapéutico no habitual de las arteriopatías obstructivas de los miembros inferiores: la cirugía limitada a la arteria femoral profunda. En 10 enfermos se realizaron 11 procedimientos operatorios: 9 endarterectomías con parche de vena y 2 angioplastias puras con parche venoso. En 6 casos se efectuó endarterectomía y parche venoso complementario en el sector inicial de la femoral superficial, para conservar su permeabilidad.

A propósito de una serie de 85 pacientes arteríticos en los cuales se efectuaron 106 amputaciones de miembros inferiores, los autores estudian la evolución inmediata y mediata de las mismas. Se hace hincapie en la frecu encia de las malas evoluciones y en la necesidad de reamputaciones. Se destaca la gravedad de las mismas en los pacientes diabéticos. La mortalidad de la serie fue del 8.24º/o.
